The season 4 finale opens with Zack making sure every detail of Fatima's baby shower is in place. Although the celebration is meant to be a eventful occasion, he cannot shake the feeling that something is wrong. After believing he spotted Leslie nearby, he quietly alerts Angela and asks everyone to stay alert without causing panic among the guests. While decorations, gifts, and laughter fill the room, an invisible sense of danger hangs over the event.
As the party gets underway, Fatima is overwhelmed by the effort everyone has put into making the day special. She thanks Zack for organizing everything before posing for photos with friends and family. In the middle of the celebrations, however, she catches a glimpse of a familiar face. Leslie appears to be blending into the crowd, and for a brief moment Fatima questions whether she really saw her. Instead of sounding the alarm, she keeps her suspicions to herself, allowing the celebration to continue while Leslie remains hidden in plain sight.
The episode then fills in the missing pieces through a flashback. Earlier, Leslie had approached Belinda under the pretense of asking for advice about hosting a baby shower. During their conversation, she cleverly managed to learn where Fatima's event would be held. After escaping custody with the help of a security guard she manipulated, Leslie made her way to the venue. To avoid drawing attention, she seemingly overpowered one of the waitresses and took her uniform, allowing her to move freely through the party without raising immediate suspicion.
One by one, several people notice Leslie's presence. Nathan spots her first, followed by Belinda, who quickly informs Angela that the woman they are looking for is the same person obsessed with harming Fatima. Zack is warned and immediately begins searching the venue, but the information spreads too slowly. Everyone is looking for Leslie, yet no one realizes that Fatima has already wandered away from the crowd.
Fatima steps away with her mother after mentioning that she thought she recognized someone. Believing there is no immediate threat, her mother briefly leaves to get her some water, leaving Fatima alone for only a few moments. Those few moments prove to be enough for Leslie to make her move she follows Fatima into the restroom, cornering her inside before anyone realizes she is missing.
Inside the restroom, Leslie unleashes months of anger and resentment. She blames Fatima for everything that has gone wrong in her life, insisting that Malachi's feelings for Fatima destroyed her future and cost her everything she wanted. Fatima tries to back away from the confrontation, retreating into one of the bathroom stalls as Leslie continues accusing her of stealing the life that should have belonged to her.
As the confrontation escalates, Fatima suddenly experiences severe pain. She looks down to discover blood running down her legs. The frightening sight leaves her frozen in shock while Leslie stares at her before leaving the restroom. Before escaping, Leslie breaks the bathroom door handle and traps Fatima inside, preventing her from getting help.
Outside, the search intensifies after someone hears a scream coming from the restroom area. Belinda points everyone toward the bathrooms, and Zack races there as quickly as he can. By the time he reaches Fatima, the damage has already been done. She is rushed to the hospital while family and friends anxiously follow behind, hoping both she and the baby will survive.
At the hospital, everyone gathers in the waiting area as doctors work to stabilize Fatima. Zack briefly steps out, his worried expression making everyone fear the worst. Before he can explain anything, a doctor calls him back inside, leaving the rest of the family waiting for answers. The episode deliberately avoids revealing Fatima's condition or whether the baby is safe, ending the medical emergency on a painful cliffhanger.
While emotions run high in the waiting room, Henry directs his anger toward Zack, arguing that he should have protected Fatima from Leslie. His accusations quickly create tension among the group. Nathan, who has often had a difficult relationship with Zack, finally steps in to defend him. He tells Henry to watch what he says, refusing to let Zack shoulder all the blame for Leslie's actions. The argument escalates into a physical confrontation before others separate the two men.
The finale closes with Fatima still inside the operating room, Zack returning to her side, and everyone left without answers about the fate of the baby. Leslie remains at large after carrying out her attack, and the season ends with multiple questions still unanswered, setting the stage for the next chapter of Zack and Fatima's story.
